I know this is a baseball play but wondered if anyone else had ever seen this----
this happened in a MLB game yesterday I was watching and never seen it before-- batter has 2 strikes and swings at a pitch-- replays showed that the ball definitely hit him in the hand and made no contact with the bat.. it also showed that he was not leaning out over the plate-- the ball went foul and the umpire called him out-- the play was not reviewed but there was a meeting of all umpires and they stuck with the original strike out call.. why not a foul ball? and what if the ball went in fair territory is it still a strike out?

Whether he gets hit by the pitch on the hand or not is irrelevant because he swung at the pitch. Had he checked his swing, he'd get first base because of the hbp.

The answer to your last question is: yes. Where the ball goes is irrelevant. It's a strike (and therefore a strikeout on two strikes). This comes up with bunts often. If the ball hits the bunter's hand the ball is not fair.

If the pitched ball first hits the batter’s hands on their attempt to hit the ball, it is a strike.
