USSSA Courtesy runner 1st inning, no outs

1st inning, Leadoff and second batters get on base safely. Batter #3 hits a single, advancing the runners to 2nd and 3rd, but pulls a muscle on his way to first base and needs a courtesy runner. Who runs for him?

We bat our entire line-up and the two guys at the end of the lineup are running late, haven't shown up yet. Is it the last guy in the lineup that is present?

Local rules don't address this scenario and I don't see anything in the 2015 USSSA rulebook either. If it's not covered anywhere, is it up to umpire's discretion?

Courtesy runners are local rules that aren't covered in the book. Around here it would be the last batter in the order, if that person isn't at the field then it's an out. Same as if a courtesy runner is due up, they are declared out as a runner and brought to the plate to hit.

Our league says the last out is your courtesy runner but they also say the batter has to get to first before being replaced with another runner.

That's kinda dumb. What are they going to appeal, that the runner missed first base? The courtesy runner's standing on it! :p

Most of the Leagues I know of if you are batting everyone and you have no outs is that the courtesy runner would be the last batter in the lineup but the runner does have to legally occupy a base before s/he can get a courtesy runner.
